Data Request Form
Please complete this online form to request data from the Office of Assessment, Accreditation, and Effectiveness. After a request is submitted, the Office will contact you within 1-2 business days to discuss the details of your request. All completed requests will be signed off by the Associate Provost. Indicative timelines for the completion of requests are stated below. During high traffic times (start of the semester, end of the semester, etc.) please allow five additional days.

Priority Level 1: Information needed for campus decision makers (Cabinet, Provost, VPs, Deans) for mission critical initiatives such as strategic planning, enrollment management, student success, etc. This may also include information needed for Middle States, NYSED, etc. that is not routine standard reports |

Priority Level 2: Information needed for specialized accreditations; grants; program approvals, evaluations, and reviews. | 7-10 |
Priority Level 3: Information requests to meet individual faculty or staff needs, such as specialized research, conference presentations, etc. | 10-14 |
Priority Level 4: General interest requests not related to college goals or initiatives. These may include ad hoc requests from external entities not affiliated with the college. |

Please note. The office is able to handle requests for research analysis including a presentation on a wide variety of topics using data science methodologies such as DFW grades effect on retention; equity gaps in student success outcomes; high school preparedness on student success, etc. For such requests completion times will vary but delivered within a reasonable time agreed upon between the requester and the office. | TBD |
